[jamming] massive potential JAM speedup

My branch of jam doesn't have make0includes() or  
make0gatherincludes().  Those functions are in Matt Armstrong's  
version.  Maybe Matt can comment on your suggestion.

Jan's optimization doesn't seem to be necessary for my branch or the  
mainline.
