[p4] Perforce dropping important stuff

I strongly recommend turning on logging on the server, if you haven't
already, and checking the logs once the problem occurs after that.
The only time I have seen something like that, and what I saw was slightly
different but equally inexplicable, occurred in the following
situation. During demo and evaluation the server was run as root, but
production level source was imported. When approvals were given and a
production setup was done, the server was run as a limited access user and
the source was simply refreshed from the eval level. Files that existed
during the eval period were still owned by root, so the server running
under more limited permissions would fail on some operations, but not
all. The fix was to change the permissions on the file tree to the user ID
that the server was running as in production.
