My personal favourite is RT:

        http://www.bestpractical.com/rt

No integration with Perforce, AFAIK.  Lots of great things about it, though.
I think that it's worth a look.

If RT didn't exist, I'd probably take a look at GNATS:

        http://www.gnu.org/software/gnats/
        http://savannah.gnu.org/projects/gnatsweb

I haven't used it myself since the early-to-mid-90s.  At the time, it had
some deficiencies that caused performance issues in large (and non-Unix)
environments but, from the rumours that I've heard, they have been addressed.

I'm sure other people with have some suggestions, too.
