[p4] changes wiped out!

A developer came to me asking why her changes were wiped out. Looking at
the file's revision history, I noticed perforce integrated changes
directly into DemographicsInspector.java #1, so the developer's change
#2 #3 were wiped out:

As far as I know, perforce should always integrate into the
head-revision, any suggestion why this is happening? Thanks,
