[p4] P4V and custom submit forms

Hi Jeff,

Its neither a bug, not ommition.

customization of submit form (change specification) 
is a very recent feature and is not matured fully
- to be reflected in p4v and
- getting the data stored in the database.

as of now, only customization of Job specification is
fully supported
