[p4] FW: Custom submission form

Another implementation would be as follows
1. Add the required fields in job
"p4 jobspec "
(modified jobspec is supported in cmdline/P4WIN/P4V )
2. have a trigger to make it mandatory to have atleast
one job attached to your changelist
+ which is in XYZ state
+ have a valid code reviewer
