[p4] Upgrading perforce from 2004.2 to the latest version

Folks,

 

We are running perforce version 2004.2 (Server version:
P4D/NTX86/2004.2/76994 (2005/03/08)) and would like to upgrade to the latest
and the greatest version.

 

Questions:

1.       What is the best way to attempt this upgrade?

2.       What are the best practices/guidelines regarding this?

3.       Any caveats? 

4.       How long does it generally take for the upgrade?

5.       Any documentation?
